Summary
To act as a focal point of communication between the GPs, practice manager, receptionist team, patients, local chemists, as well as other members of the primary health care team, to ensure patients repeat medication is produced within the current protocols of the surgery. To assist the practice to implement and maintain high quality, cost effective prescribing habits in line with current CCG and Practice prescribing policies and recommendations. To develop systems and improve the patients journey where medicines are concerned. MAIN DUTIES AND RESPONSIBILITIES: v To process, all repeat medication requests sent in by patients using the various protocols in place. v To liaise with patients and chemists regarding queries and requests v To ensure repeat medication is issued in line with the protocols that are already in place and ensure that any queries raised are dealt with quickly and correctly. v To ensure that patient queries on medication are highlighted to the relevant GP v To regularly go through prescriptions not picked up and unlikely to be picked up as a result of the timescale, to remove the details from the patient records and then destroy the prescription. v To work as part of the team providing support to the Practice to address areas within the current practice prescribing action plan. v Co-ordination and efficient operation of practice repeat prescribing systems. v To address interface prescribing issues and promote use of the Significant Event Reporting System. v To improve the quality of repeat medication records by housekeeping to align quantities, discontinue medication no longer needed and identify potential clinical issues to highlight to the pharmacist or GP. v To develop effective working relationships with relevant community pharmacists. v To help the practice achieve prescribing targets within the current prescribing Incentive scheme. v To co-ordinate medication review and develop an effective and robust recall system. v To develop good working relationships with the Wirral Medicines Management Team. v To attend ICB coordinated training events when made available. v To effectively liaise with nursing and residential care homes. v To develop, implement and co-ordinate repeat dispensing (where applicable). v To develop excellent skills in working with GP computer systems to generate reports and implement changes. v To develop written materials, for example repeat prescribing protocols, patient information leaflets and practice information charts. v To plan and implement innovative approaches to reduce medicines waste. v This list is not intended to be exhaustive and may be reviewed periodically with the post holder to ensure that the job description relates to the job being performed.
