We’re looking for a dedicated Colorectal Cancer support workerto join our Colorectal team. This is a rewarding opportunity to support patients and carers throughout their treatment journey and beyond, helping them navigate care pathways and empowering them to self-manage with confidence.
Initially this job will be based at Basingstoke, but there will also be a requirement to travel to Winchester
• Act as a key point of contact for patients, especially those on Open Access Follow-Up (OAFU).
• Deliver Level 1 psychological support and perform Holistic Needs Assessments.
• Provide telephone triage and advice within defined protocols.
• Support the planning and delivery of Health and Wellbeing programmes.
• Collaborate with Clinical Nurse Specialists (CNSs) and other healthcare professionals.
• Ensure timely and accurate documentation and communication.
• Help patients access appropriate resources and support services.
