Summary

For detailed roles and responsibilities of Clinical Site Practitioner, please see attached Job Description: Clinical & Professional Responsibilities: Provide senior nursing support and professional advice across the Trust. Offer first point of contact for ward staff during out-of-hours. Undertake advanced clinical interventions and life support skills. Liaise closely with the Outreach Team for early identification and prevention of deterioration. Perform clinical procedures and supervise ward staff in executing them. Contribute to corporate nursing agendas and maintain clinical competence. Support out-of-hours Medical Team i.e, cannulation/venepuncture, Verification of Expected Death. Management Responsibilities: Manage site operations and coordinate emergency responses. Provide senior managerial support and advice to all areas. Coordinate the Trust's response to various emergencies/incidents. Liaise with on-call teams and maintain communication standards. Facilitate staff redeployment and supervise bank/agency staff. Ensure designated shift leaders adhere to standards especially during out of hours. Act as an effective role model and demonstrate leadership skills. Collaborate with clinical site nurse practitioners for operational efficiency. Capacity Management: Lead and support daily capacity meetings for effective bed utilisation. Maintain accurate bed state and monitor patient discharges. Ensure patient movement adheres to speciality ward configurations. Facilitate patient transfers and discharge processes. Support timely patient assessment and referrals. Monitor trigger factors for A&E and capacity escalation plans. Collect and analyse capacity data for audit purposes. Education Responsibilities: Participate in staff teaching and create positive learning environments. Contribute to education and training programs for nursing and medical staff. Maintain a evidence-based approach to practice and promote person-centred care. Participate in nursing initiatives and continuing education. Take part in individual performance reviews and personal development. Clinical Governance Responsibilities: Participate in quality and risk management initiatives. Adhere to infection control safety practices and support staff in acquiring competencies. Ensure staff are supported and supervised for professional development. Identify and address own development needs and establish clinical supervision. Investigate and address incidents and complaints proactively. Participate in clinical benchmarking and contribute to quality improvement initiatives.
