Working with the clinical ward teams facilitate seamless timely discharge or transfer of patients to an agreed discharge plan. Document discharge plan in PPM+ following the board/ ward round in order to keep the patient information up to date. Work closely with internal clinical teams and external agencies to support effective patient discharge. Have an understanding of the Reason to Reside Criteria (R2R) and the codes and descriptions explaining what a patient is waiting for to be discharged from hospital. Promptly alert the Nurse in Charge/discharge facilitator of any delays or difficulties within a patients discharge plan. Promptly alert Nurse in charge to difficulties in patient flow, placement or when capacity issues arise within their responsible ward areas. Collect and maintain accurate and up to date information regarding patient flows (acute and elective admissions, discharges, transfers, delays). Ensure all patient and carer enquiries are dealt with in a friendly and professional manner. Participate and contribute to Multi-Disciplinary Team meetings or board rounds to ensure the full team understand individual patient discharge plans Communicate with patients and families their discharge plan ensuring they receive the most up to date information. Information Management Collate and enter data, on the relevant Trusts administration system, regarding patient discharge in an accurate and timely manner in accordance with local and Trust policies. Provide reports as required regarding transfers and discharges. Maintain accurate records of patients waiting for investigations and or treatment and provide reports as necessary regarding numbers of patients waiting, current waiting times and outcomes. Maintain accurate records of what patients are waiting for to go home or to another place of care . Monitor administrative processes to ensure that they meet the needs of the patients and the service. Complete mandatory training.
