An exciting opportunity has arisen for an enthusiastic, highly motivated and proactive band 8a Pharmacist with previous HIV/ID experience to work as part of the HIV pharmacy team for one year to cover maternity leave.
The specialist HIV pharmacist will have responsibility for the clinical pharmacy service delivered to HIV and an element of responsibilities to ID patients in relation to ward based pharmacy services acrossall of the Royal Free London NHS Trust sites, playing a key role in the prescribing and ongoing monitoring of HIV medicines, ensuring timely supply of HIV medicines through the out-patient pharmacy and homecare services and the day-to-day management of the HIV pharmacy team.
Potential candidates will have worked as a band 7 or 8a pharmacist with some experience working in the HIV/ID setting. They will have had some involvement in consultant ward rounds, contributing to policies/guidelines and supervision and training of staff.
Key responsibilities include:
· Regular active participation in ward rounds and complex case MDT discussions
· Delivery of a high quality, specialist ward-based and outpatient pharmacy service including medicines reconciliation, review of complex medication regimens, medicines optimisation and adverse reaction and interaction monitoring
· Training and supervision of junior members of the pharmacy team
· Timely supply of medicines for in-patients, out-patients and discharge
· Development and implementation of clinical guidelines
· Effective control of drug expenditure and management of processes regarding high-cost drugs
· Using independent prescribing skills within HIV out-patient clinics
· Counselling HIV patients on their medicines to enhance compliance
· To participate in audits and practice research
To work with the clinical risk team to minimise the risk of medicines
