Job Title: Information and Assessment Officer

Salary: £22.18 per hour PAYE with accrued holiday and pension contribution

Hours: 37 hours per week Monday-Friday

Role length: 4 months opening assignment

The role: You’ll work within Hackney’s Adult Social Care service, supporting residents and carers by providing high-quality information, advice and initial assessments. Acting as a first point of contact, you’ll deliver a strength-based approach to help people maintain independence, access equipment, Telecare, and community support. You will liaise closely with social workers and other professionals to ensure timely, effective and person-centred support.



Key Responsibilities:

Act as first contact for Adult Social Care enquiries, identifying needs and signposting appropriately.
Conduct strength-based initial conversations and non-complex assessments.
Develop outcome-focused support plans and facilitate access to universal and preventative services.
Recommend and set up Telecare and simple equipment as a Trusted Assessor.
Identify safeguarding concerns and escalate appropriately.
Maintain accurate case records and ensure up-to-date documentation.
Promote health, wellbeing, and independence across Hackney communities.



What the client is looking for:

Proven experience delivering customer service and assessment in a health or social care setting.
Strong understanding of the Care Act, safeguarding, and adult social care frameworks.
Excellent communication and interpersonal skills with diverse residents.
Ability to manage workload, prioritise effectively, and work both independently and collaboratively.
Confident IT user, familiar with case management systems and Microsoft Office tools.





Desirable:

Knowledge of Hackney’s Adult Social Care services and local community resources.
Experience working as a Trusted Assessor or with Telecare/assistive technology.
Awareness of the Mental Capacity Act and data protection legislation.
