The Integrated Urgent Care Team is a joint health and social care service which is leading on the implementation of ‘Home First’ which is Tameside and Glossop Admission Avoidance and Discharge to Assess Model.

The first part of our Urgent Care service transformation will be based around The Integrated Urgent Crisis Team (IUCT). The service strives to provide high quality community healthcare services across Tameside & Glossop. Our staff are passionate about upholding the values and principles of this revamped team, and we know that we work better together, so naturally we strive to deliver excellence through collaborative and innovative working across organisational boundaries.

To provide skilled Rapid holistic assessments for individuals referred into
the Integrated Urgent care Team, provide physiotherapy assessment, treatment and advice to adult patients in the community setting. Undertake the lead role in the assessment, diagnosis and formulation of physiotherapy treatment plans.
To advise and train patients, carers, families and staff of other agencies to ensure physiotherapy treatment is fully integrated into the patients care programme.
