SummaryAn exciting opportunity has arisen for an experienced Practice Nurse to join a progressive and supportive GP practice providing high-quality, patient-centred primary care. We serve a medium-sized patient population and consistently achieve excellent clinical outcomes and Quality and Outcomes Framework (QOF) results.
Main ResponsibilitiesWorking as part of an established multidisciplinary clinical team, you will provide comprehensive nursing care and manage patients with long-term conditions. Responsibilities include:

Chronic disease management (Diabetes, Respiratory, Cardiovascular conditions)

Health promotion, screening, and lifestyle advice

Immunisations and vaccinations across all age groups
-
Cervical cytology and women’s health
-
Travel health and sexual health advice
-
Complex wound care and treatment room duties
-
Contributing to clinical audits and service improvement initiatives

Supporting junior colleagues and sharing best practice
-
Accurate record keeping using a clinical IT system (TPP SystmOne or equivalent)

Active involvement in Primary Care Network initiatives

Person SpecificationEssential Qualifications & Experience

Registered Nurse (NMC)
-
Proven experience in General Practice Nursing

Competence in managing chronic diseases (e.g. Diabetes, Respiratory, Cardiovascular)

Trained in cervical cytology
-
Experienced in immunisations and vaccinations
-
Skilled in complex wound care
-
Familiar with electronic clinical systems
Desirable

Mentor or teaching qualification
-
Additional clinical interests or specialist skills relevant to primary care
