Summary


We are recruiting for a Palliative care CNS Band 7 to join our dynamic team.

As a specialist palliative care CNS you will provide expert palliative care nursing advice, to support and facilitate high quality, individualised care to patients and relatives/carers in collaboration with the multi professional team, in line with the local and national targets.

The successful candidate will be trained to manage own caseload of patients with appropriate input from Senior team, working as part of the multi-disciplinary Specialist Palliative Care Team. Maintain effective communication with patients, families/carers and healthcare professionals to ensure seamless service delivery.

As a valued member of the specialist palliative care team, you will play a key role in delivering holistic, patient-centred care. You will be supported to carry out comprehensive palliative care assessments, addressing physical, psychological, social, cultural, and spiritual needs. Your expertise will help ensure the provision of high-quality nursing care and inform decisions on clinical priorities.


You will also contribute to multidisciplinary discussions and, where appropriate, initiate or attend case conferences to support collaborative care planning.
• As a key member of a multi professional team, ensure appropriate referral and discussion with other members of the team to optimise patient care.
• Ensure patients are assessed, have care plans that are implemented and evaluated in accordance with Trust’s evidence based procedural documents.
• To have local open and transparent communication channels that support the multidisciplinary team to do the right thing for people we care for, to be bold when they have good ideas and to speak up when things go wrong.
• Act as a clinical expert for palliative and end of life care.
• Ensure staffing is in line with Trust policy.
• Manage budgets and resources efficiently and effectively.
• Champion a culture of learning and quality improvement within the team
• Be an ambassador for palliative and end of life care across the Trust.
