Job Description
We are seeking a qualified Occupational Therapist to join the Adult Services team at Monmouthshire County Council. This role is vital in supporting individuals to maintain independence, avoid unnecessary hospital admissions, and access appropriate care, equipment, and adaptations.

Key Responsibilities
Deliver occupational therapy interventions in patients’ homes and community settings.

Conduct comprehensive assessments and develop tailored intervention plans for individuals with complex needs.

Support admission avoidance through timely therapeutic input.

Recommend and arrange assistive equipment and home adaptations, including processing Disabled Facilities Grants (DFGs).

Carry out housing assessments and contribute to manual handling and reablement plans.

Participate in a weekly Duty rota, responding to urgent queries and visits.

Act as a Trusted Assessor to set up care packages and review existing arrangements.

Manage a varied caseload efficiently, demonstrating strong organisational and clinical reasoning skills.

✅ Candidate Requirements
Qualified Occupational Therapist with relevant social care experience.

Strong organisational and clinical reasoning skills.

Ability to work independently and collaboratively within multidisciplinary teams.

Flexibility, motivation, and enthusiasm.

Welsh language skills are desirable but not essential.

Must be able to travel within the community to visit service users in their homes.

Working Pattern
Base Location: Chepstow Community Hospital

Hours: 22.5 per week (typically 9am–5pm, 3 days per week)

Community travel required to support service users in their homes.

Participation in a Duty rota one day per week.
