An exciting opportunity has arisen for an individual to join our expanding Greater Manchester Reconnect service, in the role of Senior Practitioner.

The post holder will have excellent communication and person skills and have extensive experience of working with individuals in mental health, learning disability or criminal justice services.

Our Greater Manchester Reconnect Service is commissioned by NHS England and is a care after custody service that seeks to improve the care of people leaving prison by offering our service and support pre-release, and peer led navigation and support in the community post release.

The Greater Manchester Reconnect Service will be provided in all Greater Manchester prisons and the Greater Manchester community.

The post holder will work within a multi-disciplinary team responsible for supporting individuals leaving the prison setting to enhance their health, wellbeing and quality of life, assisting them to connect with and access health services, voluntary sector services and community-based activities and support.

The post holder will work agilely across the GM footprint to:
1. Provide supervision, support and advice to junior staff in the team, in line with relevant clinical pathways as outlined within the operational procedures.
2. Manage and evaluate a caseload in line with relevant pathways as outlined within the operational procedure and will include: -
• Assessment
• Formulation
• Signposting
• Close working with all colleagues
• To support with complex cases
