North (Covering North Herts and Stevenage)
Core service hours (Mon-Thurs 9am-5.30pn and Fri 9am-4.30pm)
Hybrid working
Max £34 p/hour (depending on level of experience)
Interviews will be via MS Teams.
• Based in the locality – covering relevant areas
• Supporting reducing the risk of admission to hospital
• Assessing/reviewing individuals in line with the Care Act
• Arranging appropriate care and support
• Undertaking Safeguarding enquiry officer role
• Completing Mental Capacity and BI decisions
• Working with other professionals

Degree/Masters in Social Work and SWE registration.
2+ years of post-qualification experience.
Strong knowledge of UK social care legislation and practice.
