An exciting opportunity has arisen within our Stroke Unit for a dynamic, committed, compassionate and highly enthusiastic staff nurse to join a hardworking and close-knit team on West Raynham.

We are looking for Band 6 Adult Nurses who want to build on their development in their career on an Acute Stroke Ward. There is an expectation that the successful candidate will work within the wider multidisciplinary team and support the delivery of stroke specialist care in accordance with local and national agreed protocols defined in SSNAP.

The successful candidate will also be expected to work within the Hyper Acute Stroke Unit bay, providing specialist stroke care.


West Raynham Ward in Queen Elizabeth Hospital are passionate about getting it right for patients and their families. In order to achieve this we are looking to appoint passionate, enthusiastic, motivated and dynamic nurses who are ready to take the next step to develop their leadership and management skills.
• Communicate with patients, relatives, carers and members of the multidisciplinary team – by telephone, face to face, in writing or electronically to ensure that discharge takes place in a timely fashion.
• Act as a resource to ward staff offering advice and help on all aspects of discharge planning, prioritising complex cases.
• Develop and maintain effective relationships with other agencies involved in discharge ensuring they are involved at the earliest opportunity in the discharge of patients with complex needs.
• Attend case conferences as appropriate, giving advice / input as required.
• Attend daily progress meetings with Social Service and PCT discharge team representatives.
• Maintain accurate records in relation to individual patient discharge.
• Ensure appropriate information is available to patients, relatives and carers in order for them to understand the discharge planning process.
• Contribute to effective communication and liaison within Discharge Planning Team supporting a unified approach to service development.
• Act as an advocate for patients when appropriate in ensuring their discharges are safe and effective.
• Respect confidentiality regarding all information received relating to patients, employment, other members of staff and the general public. Uphold the requirements of the Data Protection Act.
