This is a fantastic opportunity to play a vital part in shaping how care and support come together across Blaenau Gwent. As a Community Multidisciplinary Coordinator, you’ll help build a seamless, person-centred approach to wellbeing — connecting professionals, improving communication, and ensuring citizens receive the right care, in the right place, at the right time.
About the Role
As a Community Multidisciplinary Coordinator, you’ll play a key role in bringing health, social care, and third-sector professionals together to plan and deliver coordinated, person-centred support for our citizens.
You’ll organise and lead multidisciplinary team (MDT) meetings — preparing agendas, capturing actions, and ensuring follow-up tasks are completed. You’ll be the vital link that keeps communication flowing between partners, ensuring that decisions made in meetings translate into timely, practical support for citizens.
Alongside this, you’ll help monitor performance, analyse data, and identify opportunities to enhance the way we work. You’ll be encouraged to use your initiative, bring fresh ideas, and help shape a more connected, efficient, and compassionate community service.

About You
We’re looking for someone who’s organised, confident, and cares deeply about helping others. You’ll be the kind of person who enjoys bringing people together, keeping things running smoothly, and finding positive solutions when challenges arise.
If you’re great at juggling priorities, communicating clearly, and staying calm under pressure, you’ll thrive in this role. You’ll use systems such as Microsoft 365 and Teams to support the smooth running of meetings and information sharing, so being comfortable with technology will be a big advantage — but what really matters is your positive attitude and willingness to learn.
Experience in health, social care, or community work would be an advantage, but it’s your compassion, teamwork, and drive to make a difference that will set you apart.
