The post holder will be working under direct or indirect supervision as a trainee enhanced or intermediate clinical practitioner in the delivery of high-quality clinical care delegating clinical decisions to enhanced/advanced practitioners or the general practitioner team. The post holder will start to build a portfolio of evidence within clinical practice and support, nursing and allied health teams to improve the patient experience, supporting the trainee and advanced clinical practitioner, service lead and medical and nursing team with day-to-day clinical activity within the Frailty team. Working in the UCR team you will respond to the Frailty GP's request for home visits, in a timely manner and within a 2-hour response time and provide clinical assessment of the patient in their own home, following this you will debrief to the Frailty hub GP's or Nurse consultant who will support you to make a differential diagnosis, management and treatment plan. The post holder will ensure they complete the relevant competencies, skills and training for the role as per the skills matrix and engage in monthly combined, clinical and educational supervision. Supporting the designated workspace areas to deliver consistent high quality clinical care with both on-going clinical care provision, ensuring patients are in the right place at the right time with the right care. There may be a requirement to provide cross site or alternative site cover dependent on operational need. Core Functions The post holder will demonstrate the knowledge and skills of an intermediate clinical practitioner and will: With delegated direct or indirect support complete clinical assessment and care for patients. Under supervision will complete complex frailty, physical and mental health assessments and form differential diagnoses and appropriate treatment. With guidance from enhanced/advanced or general practitioners will prescribe medication within scope of practice as appropriate (however this is not currently a requirement for the role) select and order appropriate investigations independently. Help to Identify the need for improvement and change within the area of responsibility and following agreement and approval, support with the implementation and development of new policies, procedures and practices and where appropriate with support implement service re-design solutions to better meet the needs of the patients and the service. Travel or working across or at alternative sites for both clinical and supervisory duties may be required. Administrative Responsibilities Ensure that medical and patient records are comprehensive, accurate, and up-to-date. Complete coding accurately, providing a clear explanation of the patients condition and treatment plan. Maintain accurate and timely documentation of patient care, including information for referral purposes. Will support and on delegated direction completion of ReSPECT forms and advanced care planning, tailored to the patients level of frailty, clinical condition, co-morbidities and wishes. Ensure that conversations with the patient and family members are properly recorded and discussed as appropriate. Clinical Responsibilities With support as required perform clinical assessment and feed back to the advanced clinical practitioner or general practitioner to enable them to form a differential diagnose and plan. Support with the implementation of the plan, and evaluate individual treatment plans, interventions, and care packages for patients, ensuring safe and appropriate management, timely referrals, and safety netting as required. Collaborate with the multidisciplinary team (MDT) to expedite safe and timely triage consultations, interventions, and reviews. If appropriate and competent prescribe within scope of practice with support as required, review medication for therapeutic effectiveness, and ensure treatment is tailored to patient needs, adhering to contraindications, drug interactions, evidence-based practice, national and practice. protocols, and within scope of practice to optimise outcomes. Prioritise and manage personal workloads, embedding effective time-management strategies. Maintain effective communication within the MDT ensuring active participation with team members and facilitating clear communication across roles. Complete clinical skills in alignment with the skills, knowledge and competence matrix for intermediate or trainee enhanced practitioners. Support patients and families receiving "bad news," engaging the team as needed to provide appropriate emotional support. Act as an advocate for patients and colleagues, ensuring needs and concerns are effectively represented. Take responsibility for personal learning and professional development and maintaining a personal professional portfolio to demonstrate ongoing growth. Work with direction from the enhanced and advanced clinical practitioner and advanced practice lead to implement the workforce strategy for Nimbuscare. Provide support to nursing staff, therapists, and students within the service. Responsible for assessing ongoing care needs and planning effective transitions, ensuring continuity and quality of care. Ensure clear communication of care plans to the team, fostering collaboration and consistency. Operate diagnostic equipment to monitor and assess patients, ensuring accurate readings for informed clinical decisions. Identify and report faulty equipment, ensuring it is removed from clinical areas for repair to maintain safety. Maintain up-to-date knowledge within scope of practice. Accountable for professional actions. Developing the theoretical and practical knowledge necessary to deliver job responsibilities to a high standard with delegation and supervision. Serve as a role model across teams and departments to ensure evidence-based practice, meet standards, and improve patient outcomes and family/carer satisfaction. Participate in clinical and educational supervision. Manage and prioritise own workload according to patient needs and service requirements. Help to Improve and maintain staff morale through effective communication, leadership, optimism, and consultation, especially in complex situations, in line with policy and guidance. Contribute to the development of patient information to support patient care. Actively contribute to the clinical governance agenda, ensuring compliance with statutory and mandatory training. Have knowledge of policy and practice changes to improve service delivery. Lead by example in fostering an inquisitive approach and driving a culture change within clinical practice, in line with the patient safety incident response framework, to enable the delivery of excellent person-centred care and support for families/carers. Participate fully in meetings, clinical supervision, and other key meetings. Research and Audit Responsibilities Supports with audits within clinical area. Utilise research findings to inform and enhance clinical practice. Support with data collection for service evaluation and audit. Identify and address personal development needs, staying current with new developments in the field. Education and Training Support with the development and implementation of educational programmes for patients and carer's, providing essential information on treatment plans and care procedures to promote engagement and understanding. Work with the enhanced clinical practitioner and the trainee and advanced clinical practitioner, lead of service, advanced practice lead, general practitioners, and nurse consultant to provide clinical guidance and educational support to junior staff, ensuring continuous learning and high-quality care. Develop skills in delivering training sessions for healthcare workers, enhancing skills and knowledge to improve care delivery across Nimbuscare's services.
