Main Duties and Responsibilities As a Band 6 Multidisciplinary Pathway Worker, your primary focus will be on ensuring timely and safe care for young people. Your key responsibilities will include: Serving as the first point of contact bycovering the duty lineto triage needs and provide immediate support. Undertaking comprehensiveinitial assessmentsfor new referrals to understand complex presentations and determine the appropriate clinical pathway. Proactively conductingclinical harm reviewsfor young people on the waiting list to ensure their ongoing safety and well-being. Performing detailedrisk assessmentsand devising effectivesafety plansto manage any potential harm. Collaborating closely with the MDT, presenting your assessments and contributing to discussions to formulate and coordinate effective, collective care plans. Utilising theRIO patient record systemto maintain accurate and timely clinical notes in line with trust policies.
