We are seeking a passionate and experienced individual to join a dynamic community mental health team. Main Duties and Responsibilities: Manage a defined caseload with autonomy, taking responsibility for the assessment, treatment, and discharge of adults with complex mental health needs. Conduct comprehensive, evidence-based assessments and develop, implement, and evaluate effective, person-centred care plans. Provide a range of specialist therapeutic interventions to support recovery and promote well-being. Offer clinical supervision, specialist advice, and support to junior colleagues and multi-agency partners. Work collaboratively within a Multi-Disciplinary Team (MDT) to ensure the delivery of integrated, coordinated care. Act as a Care Coordinator under the CPA framework, ensuring high standards of record-keeping and communication. Uphold the principles of safeguarding, ensuring the welfare of vulnerable adults at all times.
