Service/Ward/Department Summary:
This is an exciting time to join the team at Pathfinder Stockport. We provide a non-discriminatory and user friendly service, offering recovery focussed interventions to reduce substance misuse related harm for individuals, their families, and the local community. We work in partnership with wider health and wellbeing services to meet the diverse needs of Stockport residents, and to deliver a high standard of care.
All new starters to Pennine Care NHS Foundation Trust who are appointed to a post which requires a Disclosure Barring Certificate check (DBS) will be recharged for the full cost of the check. Substantive staff will have the amount deducted from their first months’ salary.
Role Summary:
A substance misuse worker, providing recovery focussed interventions to people whose alcohol and drug misuse has resulted in a range of offending behaviour.
Key Responsibilities of the Post:
• To actively manage the recovery journeys of a case load of drug and alcohol clients with complex needs
• To actively manage a caseload of clients receiving OST
• To support both drug and alcohol clients through community and in-patient detoxification programs.
• To formulate, implement and review care plans.
• To provide reports in relation to Child and Adult Safeguarding and attend Child In Need and other safeguarding meetings
Key Requirements of the Person Specification:
• Experience of working with problematic drug and alcohol users
• Experience of managing a caseload of clients receiving OST
• Experience of working in a health/social care setting
• Experience of multi-disciplinary working
