Hours: 37 hours per week

Based: Home and field based (focus on Nottingham City)

About the role
Are you passionate about empowering people to live independently and improve their wellbeing?
We’re looking for a Community Support Worker to join our Wellbeing at Home service – our short-term community support service is designed to help adults maintain independence and improve health and wellbeing.
As a Support Worker, you will:
Provide practical and emotional support to individuals to help them achieve their goals.
Work collaboratively with service users to develop and follow short-term support plans.
Promote independence, wellbeing, and social inclusion.
Support individuals to access community services, benefits, housing, and healthcare.
Maintain accurate records and contribute to service monitoring and evaluation.
About you
We’re looking for someone who is:
Has experience in a support or care role (paid or voluntary).
Communicates effectively and builds trust quickly.
Is organised, reliable, and able to work independently.
Has a good knowledge of local services and resources (or is willing to learn).
Is able to work in a hybrid way, travelling across Nottingham City as required.
