Crynodeb
This is a small team of 4 B6 Nurses and a B7 Senior Practitioner. We work Mon - Fri 9-5. There is a daily morning handover, and weekly formulation meeting. Visits are planned with the team in advance, with some urgent slots available. The role is to: Undertake specialist mental health assessments Determine the nature & severity of mental health needs and provide sign posting and advice Provide support & advice to care home/ domiciliary staff / relatives/ carers/ support networks. Contribute to, implement and review care plans in line with the service user's needs Provide education & training for care home/ domiciliary staff / relatives/ carers/ support networks. To work within the Mental Health Act framework, and liaise with appropriate professionals and services such as AMHPs, consultants, hospital wards, when needed.
