We are looking to appoint at Grade 8 level to be part of an established team of social workers within the High Needs Team in Adult Social Care.
You will work as part of an integrated multi-disciplinary team to provide high quality social work practice. You will ensure individuals have a solution focussed, strength-based plan towards achieving their goals and maintaining their independence, throughout their journey in adult social care.
You will work creatively and collaboratively with (in some cases from the age of 16) with our partners in Children’s services. This is to provide a consistent approach within a social work interface in preparation for their transitioning journey into adult’s services.
You will be working in a Multi-disciplinary arena which will include colleagues in the Community Learning Disability Team, Housing, Criminal Justice, Probation, Community Mental health and other health Professionals.
We are looking for individuals who are qualified and experienced social workers who can demonstrate:

A strong value base that informs your work with young persons and adults.
A continuing commitment to quality outcomes - and to be able to talk about what good Social Work practice 'looks like'
A capacity for honest reflection and openness to learning;
Ability to establish professional, effective working relationships with a range of partners/colleagues, adults, families and their carers
The ability to work in a multidisciplinary arena to promote autonomy and implement personalised safeguarding
Understand the complexities involved with vulnerable persons
You must be a qualified social worker and be registered with Social Work England.

This post is for you if you have completed your ASYE and can demonstrate increased decision making autonomy in situations of increasing complexity, risk, uncertainty and challenge.
The High Needs Team provides consistent and intensive case management during the initial stages of a service being set up or where the professional view is that support can be reduced over a period of time e.g. over the first years of care. It provides a level of assurance for the management team (DMT) in terms of having oversight across packages of support which may involve a community deprivation of liberty and Standard Authorisation, there is a requirement to regularly review the restrictions which are in place to keep the person safe.
