Detailed job description and main responsibilities The job holder will assist Neonatal Unit, transitional care based staff with early and appropriate discharge planning in accordance with the criteria for outreach care. To provide a seamless timely discharge from hospital to the home environment. The job holder will carry continuing responsibility for a defined caseload, including the assessment of care needs and the development, implementation and evaluation of complex care programmes, ensuring continuous provision of integrated care. To provide on-going holistic care within the community setting for vulnerable neonates and their family. Provide specialist on-going care for the neonate and education, advice and support to the family in the home environment. The job holder will take a proactive role in the education and training of the multidisciplinary team(s), providing on-going supervision and support as an expert. Resource for all disciplines where neonates require on- going care needs within neonatal outreach. To work effectively and efficiently as an Outreach team member as well as independently in the planning, assessment, implementation and evaluation of neonatal community care. To plan, time manage and initiate a weekly schedule based on workload requirements, which will meet the needs of the service and ensure that individual family needs are met.
