Crynodeb
The post holder will provide Support, give Time to an allocated group of service users and promote their recovery. To assist the Care Coordinator and the Multi-disciplinary team to assess, plan, implement and evaluate individual care plans. The purpose of this post is to work as part of a team focusing on the direct needs of service users working across boundaries of care and organisations and use the agreed set of values and skills to underpin their day-to-day work. To provide a flexible working pattern that meets the needs of service users.
Please note - This role is not eligible for sponsorship unless you meet criteria by temporary exemption from current changes to immigration rules put in force on 22/07/2025. Separate provisions are applied to workers who have been sponsored and held continuously Skilled Worker visa since prior to 04/04/2024.
To be responsible for contributing to the Step-Down Process of service users in the community. This will include attending and actively participating in MDT’s, training sessions, care plan review meetings and care coordinator as required.
Under supervision from the MDT, carry out prescribed care as per an agreed care plan to meet the individual needs of service users.
· To assist in the development and preparation of activities that conforms to the goals of an agreed care plan.
· To positively promote independent living within the community and make activities responsive to the needs of the service user
· To provide support with daily living activities which promote social inclusion and recovery
