Summary
Job purpose To work as a senior dietitian, providing evidence-based nutritional information and support to patients and their families across primary and secondary care. Manage a delegated caseload of paediatric patients with a variety of acute and chronic diseases with clinical support from specialist paediatric dietitian. Ensure their treatment is evidence based and patient centred. Assess, plan and implement interventions. Clinical caseload: Acute paediatric ward, Weight management, faltering growth, non- IgE mediated allergy Job specific outcome Job specific outcomes Act as an independent practitioner and be professionally and legally accountable and responsible for all aspects of their own work. Provide Dietetic treatment to a delegated caseload using evidence-based patient centred principles to assess, implement and evaluate interventions. Interpret medical, biochemical and pharmacological information, and incorporate into dietetic plans as appropriate. Recommend and discuss treatment options with the Consultant Paediatricians/General Practitioners / Referring Agents/ specialist nurses. Develop nutritional care plans for a caseload of patients based on assessment of medical, social, cultural and psychological factors. Monitor and review the aims and targets. Act as a resource on aspects of general nutrition to patients, carers, other health and education professionals, including GPs, consultants, and student dietitians. To cover patients from the Dietetic team based with as caseload requires Liaise with supervising/Specialist Dietitians regarding complex patients to agree appropriate management. When appropriate liaise with members of the health care team and/or specialist Dietitians regarding further management of patients in your caseload once they have been discharged from your Dietetic care. Decide priorities for own work, balancing other patient related and professional demands in consultation with the Head of Nutrition & Dietetics. Recommend prescription of oral and enteral nutritional products to medical practitioners in primary and secondary care on behalf of patients and advise on their safe usage. Contribute to the delivery of education programmes to staff and patients within delegated caseload. One to one dietetic counselling, as well as group education to staff, patients and their families will be included in this post. To participate in dietetic/MDT/ specialist team meetings and case conferences to discuss the nutritional management of patients. Contribute to the training and development of Dietetic students and Dietetic Assistants. Support senior staff with service developments and input into policy, guideline and procedure development as well as departmental audits. This will include updating and development of nutrition and diet literature and resources and to ensure this is nonbiased and evidence based /best practice. To attend and participate in regular dietetic team meetings. To deliver dietetic care in accordance with Nutrition & Dietetic service standard, organisational policies, clinical governance, National Professional Standards of Practice and the Health & Care Professions Council (HCPC) Standards of Conduct, Performance and Ethics. Maintain appropriate, accurate and timely patient records in line with Nutrition & Dietetic service policy, GDPR, HCPC, BDA and Jersey Data Protection Law. Statutory responsibilities Active engagement, participation and compliance with any other statutory responsibilities applicable to the role, as amended from time to time. The post holder will have to comply with all relevant States Laws, such as the Health and Safety at Work (Jersey) Law 1989 and any other associated legislation, Standing Orders, Financial Directions and other Codes of Practice. To work in accordance with the Data Protection (Jersey) Law. Ensure appropriate communication of relevant patient information through the maintenance and updating of patient/client records in line with requirements of Clinical Governance and Quality as set out in the Standards of Practice from BDA; HCPC and HCS. To meet all requirements of Clinical Governance (e.g. statutory & mandatory; CPD; reflective practice etc.) and propose any necessary changes to dietetic service delivery which this may give rise to. To interpret and inform the relevant MDT of specific national and professional evidence and policies which affect the specialist area. This role is politically restricted. The job holder is not permitted to undertake political activity involving standing for election to the Government or as a Parish Constable, or publicly supporting someone who is standing for election or playing a public part in any political manner. Specific to the role Qualifications- ESSENTIAL-BSc Hons Degree in Human Nutrition and Dietetics or Postgraduate Diploma/ MSc in Human Nutrition and Dietetics. Registration with the Health Care Professions Council (HCPC) and British Dietetic Association (BDA). Evidence of Continuing Professional Development DESIRABLE-Accredited post registration training in relevant areas. Membership to BDA Specialist Interest group. Knowledge ESSENTIAL-Accepts responsibility and accountability for own work and can define the responsibilities of others. Recognises the limits of own authority within the role and seeks professional support appropriately. Demonstrates Codes of Conduct and Professional Standards of the Health Care Professions Council (HCPC) and British Dietetic Association (BDA). Seeks and uses professional support appropriately. Ability to employ evidence-based monitoring and evaluation approaches and effectively communicate findings and recommendations to decision makers DESIRABLE-A clean driving Licence and access to a vehicle. Self-motivated and able to demonstrate initiative Experience ESSENTIAL-Experience of working as a general or junior dietitian, across a variety of clinical settings and specialities. Demonstrates understanding and experience in the ethical and moral dimensions of practice. Experience of working effectively within a multidisciplinary team. DESIRABLE-Experience of research or audit. Experience in teaching and training other healthcare providers. Experience of dietetic student training. Criteria relating to Safeguarding ESSENTIAL-Registration as a Dietitian with the HCPC and Jersey Care Commission. Current DBS enhanced check and up to date adult safeguarding training/child protection training. Specific to the role Provide nutritional counselling involving a range of therapeutic diets. A high degree of motivational, negotiating and reassurance skills are required. May involve high sensitive information and communication through interpreters. Translate highly complex scientific, medical, biochemical and nutritional information into easily understood practical advice for individuals and groups. Frequently needs to be able to communicate information clearly and sensitively in what can sometimes be an emotive atmosphere. Work collaboratively with members of the MDT and act as a source of clinical specialist advice. Uses advanced communication skills as educator to other professionals including Doctors, Nurses, AHPs and students. Analytical Skills Understands and analyses patient information using specialist assessment to provide appropriate therapeutic management plans. Make recommendations to the MDT where expert opinion may differ. Produce/revise treatment plans within a range of highly complex dietetic problems. Interpretation of biochemistry within specialist clinical area covered. Address patients/carers where there are issues around neglect/ abuse/ family dynamics. Prescribe ACBS endorsed nutritional products and monitor the appropriate use of these. Participate in audits as part of the Nutrition & Dietetic service. Planning & Organisation Plans and balances competing and conflicting priorities. Follows agreed methodologies in line with the research-base of what works to achieve outcomes. Professionally and legally accountable for own caseload and works autonomously. Provides clinical supervision to dietetic students. Liaise with administration staff to plan and manage clinical services. Physical Skills Will use keyboard, mouse and PC on most days and normally for periods between 3-6 hours. Audio-visual equipment for education/ training. Will likely be required to set up equipment for group education including laptop, food models, flipcharts etc. Ability to use anthropometric equipment i.e. scales, height stadiometers, skin callipers, hand grip strength devices. Telecommunications for internal and external communication. Have a valid driving licence and preferably access to a car for work purposes. Good keyboard skills for use of relevant IT systems
