Summary
What we ask from you? - Working in a multi-disciplinary team alongside Clinical and Operational Leadership to ensure high quality care delivery to service users. - Provide specialist assessment and care planning in conjunction with service users and carers. - Evaluate treatment plans and undertake discharge planning and promote self care. - Be responsible for an agreed caseload. - Support development of skills and competencies in others.
