Summary
As a Specialist Practitioner, you will be a Registered Nurse or Paramedic with the skills to work autonomously to provide a high-quality telephone triage and home visiting to patients referred to the GP Out of Hours Service. You will be required to attend patients’ home to assess and treat patients with acute primary care illness needs. You will have excellent clinical assessment and decision-making skills along with a passion to provide patient centred care.
You will be joining a multi-disciplinary team who pride themselves on their excellent team working and support of each other whilst treating a diverse range of conditions which will give you the opportunity to extend your advanced skills in illness.
You will be working across a shift system which will be weekends, bank holidays and nights. We are happy to consider full or part time hours on a permanent basis. You will work from an Out of Hours base. We have bases in Oxford, Banbury, Abingdon and Witney for home visiting.
The service operates 365 days a year, and the post holder will work flexibly across shift patterns.
Main duties of the jobThe post holder will provide holistic patient care within the unscheduled care setting, inclusive of the patient’s residence, from assessment of presenting condition through to diagnosis, initial treatment and discharge, including the administration of medication within agreed clinical guidelines (PGDs/Eolas) and protocols.
The post holder will be expected to complete telephone consultations when not home visiting whilst on shift.

- The post holder will demonstrate a commitment to providing a quality, evidence-based service, excellence in clinical practice, this together with continuous professional development are integral to the role and development of the service.
- To work in close partnership and liaison with primary care, acute and specialist services, Urgent Community Response (UCR), Hospital at Home, Single Point of Access (SPA), Community Services, social services, mental health services and voluntary services both clinically and in relation to service development.
- To complete contemporaneous clinical documentation whilst home visiting using the software available 3.
- Post-registration qualification in Minor Illness (Level 7).
- Mentorship qualification or equivalent.
- Advanced Clinical Assessment Qualification.
- Registered General Nurse or Allied Healthcare Professional.
- V300 Non-Medical Prescribing.
- Proficient in catheterisation – male and female.
- Competent in using Evidence Based Practice.
- Proficient IT skills.
- Palliative care and syringe driver management.
- Suprapubic catheterisation.
- Experience at Band 7 equivalent in assessment of minor illness presentations in the home environment.
- Experience in telephone triage within primary care for acute illness presentation.
- Experience in management of palliative care/End of life patients in the home.
