Summary

Carer Support Furness are looking for a self-motivated and enthusiastic individual for the post of Carers Support Services Adviser. This is an exciting opportunity to work within a recognised local Charity who support unpaid Carers. The main tasks and responsibilities of the post will be to undertake unpaid Carers Assessments (All Age Carers), provide information and support, raise awareness of unpaid Carers needs and to assist with the identification of unpaid Carers.

JOB PURPOSE:
• To work as part of Carer Support Furness, Carers Support Services Team and assist them to identify and assess the needs of unpaid Carers who are looking after a Family Member, Partner or Friend.
• To complete Carers Needs Assessments and manage caseload. This role is for supporting unpaid Adult Carers, Parent Carers and Young Carers.
• To complete Carers Reviews and manage caseload.
• To complete Carers Contingency Plans and manage caseload.
• To complete Carers Support Plans and manage caseload.
• To facilitate and deliver Carer Awareness sessions (when requested).
• To work with Health & Social Care locality teams
• To ensure that unpaid Carers, members of the public are able to access support services to meet the identified individual Carers needs.

MAIN TASKS & RESPONSIBILITIES:
• Raise awareness of the needs of unpaid Carers of all ages and the issues relating to their caring role.
• Identify unpaid Carers and undertake a formal Carers Needs Assessment in accordance with the assessment criteria of the local authority.
• Assess individual unpaid Carers needs for support services to develop a support plan and link them to appropriate services to meet identified needs.
• To complete contingency plans and risk assessments as identified through the Carers Needs Assessment process.
• Establish and maintain accurate records to ensure efficient documentation of all areas of support services offered and provide data (qualitative and quantitative) to be used for monitoring progress, identifying unmet need and influencing future planning of services offered to unpaid Carers.
• To record and input Carers Needs Assessments, Carers Contingency Plans, Carers Support Plans and Carers Reviews onto the dedicated IT systems as defined by the local authority.
• Develop and maintain links with health & social care locality teams and other agencies to raise awareness of unpaid Carers issues and their needs.
• Exchange information and consult with health & social care locality teams and other third sector organisations to ensure the delivery of appropriate services for unpaid Carers.
• When requested to attend meetings and provide written progress, monitoring and evaluation reports to ensure the development of Carers support services.
• To attend team meetings both at Carer Support Furness and within health & social care locality teams.
• To attend regular supervision sessions and report back any concerns, problems or issues.
• When requested to work with colleagues within Carer Support Furness to ensure information, advice and support services is delivered to the highest standard to unpaid Carers of all ages and that the work of Carer Support Furness is fully complementary.
• To undertake other duties as may be required with the position.
