Description
We are seeking a dedicated and compassionate Outreach Support Worker (Scale 5) to join our dynamic team. Responsibilities
Conduct outreach activities to engage with clients in the community.
Assess the needs of clients through interviews and observations.
Develop personalized support plans tailored to individual needs.
Connect clients with necessary services, including housing, health care, and employment resources.
Provide ongoing support and guidance to clients, advocating for their needs.
Monitor clients' progress and adjust support plans as necessary.
Collaborate with social service agencies and other organizations to facilitate comprehensive care.
Requirements
Bachelor's degree in social work, psychology, or a related field.
Experience in outreach or case management roles preferred.
Strong interpersonal and communication skills.
Ability to work independently and as part of a team.
Knowledge of community resources and social services.
Demonstrated ability to engage and build rapport with diverse populations.
Valid driver's license and willingness to travel within the community.
Hours Per Week: 36.00
Start Time: 10:00
End Time: 19:30
Pay Per Hour: £13.39
WK 1 - Tuesday 11-7pm, Thursday 9-5pm and Sunday 10-6pm
WK 2- Wednesday , Thursday and Friday 10.45am - 6.45pm
WK 3 - Tuesday, Wednesday and Thursdays 11am -7pm
WK 4 - Wednesday, Saturday and Sunday 10-6pm
