Description
We are seeking a dedicated and compassionate Outreach Support Worker (Scale 5) to join our dynamic team. In this role, you will play a critical part in providing support to individuals in the community who may be facing various challenges, including mental health issues, homelessness, substance abuse, or social isolation. As an Outreach Support Worker, you will engage directly with clients in their environments, fostering trust and building meaningful relationships. You will collaborate closely with various stakeholders, including healthcare professionals, community organizations, and social services, to ensure a holistic approach is taken to meet each client's unique needs. Responsibilities
Conduct outreach activities to engage with individuals in the community in need of support.
Develop and implement individualized support plans in collaboration with clients to address their unique needs.
Assist clients in accessing relevant services, including healthcare, housing, employment, and educational opportunities.
Maintain accurate and confidential records of client interactions and support progress.
Facilitate workshops and support groups that promote skill development and community engagement.
Collaborate with multidisciplinary teams to ensure comprehensive support and care for clients.
Advocate for clients’ rights and needs within the community and with relevant agencies.
Monitor and evaluate the effectiveness of interventions and recommend adjustments as needed.
Requirements
A relevant qualification in social work, psychology, or a related field.
Experience in outreach work or community support roles is highly desirable.
Strong interpersonal and communication skills to engage effectively with diverse populations.
Ability to work independently and manage time effectively in a fast-paced environment.
Demonstrated understanding of the challenges faced by at-risk individuals, including mental health issues and homelessness.
Knowledge of local community resources and support services available to clients.
Commitment to maintaining client confidentiality and upholding ethical standards in all interactions.
Hours Per Week: 35.00
Start Time: 09:00
End Time: 17:30
Pay Per Hour: £13.39
