Summary


The Ipswich Hospital NHS Trust has an exciting opening in the Diabetes Team.

The Diabetes team is dynamic and forward thinking and have recently expanded to provide an integrated diabetes service for East Suffolk. The team has won several awards including the Quality in Diabetes Care award on 2 occasions, a Univants award for our peri operative pathway and the HSJ award.

We now have an exciting opportunity for a Band 6 Diabetes Specialist Nurse to join our team. You will be based at the Diabetes and Endocrine Centre working as part of this multidisciplinary team, participating in the delivery of integrated diabetes care for people with diabetes across East Suffolk.

This post is mainly based in the in-patient setting across surgery and medicine with close working relationships with our out-patient and community-based nursing team colleagues.

We offer secondary care services to patients with both Type 1 and Type 2 diabetes as well as covering specialist areas of Diabetes in pregnancy, Diabetes related renal disease and also have a substantial amount of patients using insulin pump therapy. The in-patient nurses will therefore need to have a working knowledge of the acute management of diabetes in all the specialist areas involved. We are looking for an enthusiastic, motivated individual to join us. • To work in collaboration with other health care professionals to develop new and innovative tools to improve patient care
• To work in collaboration with patients and their carers to develop individualised care plans providing knowledge and support enabling them to self manage their diabetes
• To provide support for all grades of nursing staff in relation to diabetes nursing and to ensure asafe framework for practice is in place and maintained
• To work with patients in a variety of settings (ie clinics, wards and community) providing a service that is sensitive to the needs of the individual
• To have Specialist knowledge regarding the actions of both oral and injectable medications used to treat Diabetes, ensuring safe practice in the hospital setting
• To act as Specialist resource
• To deal with distressing/emotional circumstances related to patients/relatives and carers
