Please see attached job description for full details Key Duties and Responsibilities Urgent Care and On-Call Provide same-day clinical consultations for urgent presentations -face-to-face, telephone, or online Participate in the on call rota in rotation with other clinicians Work collaboratively with the on-call team and administrative staff to ensure safe and timely patient care Home Visiting Undertake same-day or pre-planned home visits, particularly for frail, palliative, or complex patients Liaise with community and multidisciplinary teams to ensure continuity of care Contribute to end-of-life care planning and prescribing Care Homes Provide medical support to allocated care homes as part of a coordinated care team Attend regular ward rounds and contribute to proactive care planning and medication reviews Build effective relationships with care home staff and allied health professionals QOF and Contractual Target Delivery Support the practice in delivering QOF targets and other contractually mandated services Engage in chronic disease management clinics, audits, and structured reviews where required Contribute to clinical coding and data quality improvement initiatives Additional Duties Work in partnership with Nurse Practitioners, Physician Associates, Paramedics, and other team members Attend clinical and practice meetings, contributing to the development of services and protocols Participate in supervision, CPD, appraisals Promote a culture of learning, safety, and quality improvement
