An exciting new position has arisen for an Intensive Support Nurse to work with individuals who have a learning disability, with additional complex and high risk needs, who may require a crisis intervention and then continual intensive support to prevent placement breakdown, an admission to hospital or following discharge from an inpatient setting where required.

You will manage an active caseload of the individuals within our care and also
support the discharge process to ensure it is both smooth and timely.

You will directly work with other outside professional, service providers, partner agencies and local authorities to support s.117 planning.

To provide intensive clinical nursing advice and therapeutic interventions to individuals within our care who present with challenging behaviour, autism, mental health and highly complex needs.

To focus on individual health outcomes for individuals within our care and offer
support and guidance to access health services.

To work with the individual, their carers and members of the multi professional teams in the development, implementation and review of a CPA and/or a person centred plan. This will include treatment plans that will outline the support the individual requires to continue to live successfully within the community.

As part of the team at Your Support Services you will work closely and collaboratively with the wider MDT to support adults within our supported living properties and within the community of Birmingham.

Your Support Services is proud to value themselves as a person-centered service, that works closely with the patient and family towards understanding and supporting the need of the individual in the least restrictive way. The service offers a variety of interventions and offers personalised plans for the service user, which can then be in-cooperated into the individual’s Positive Behaviour Support plan.

You will be an experienced Nurse or Newly qualified nurse, flexible with excellent communication skills and a passion for integrated team working. With a registration in RNMH/RNLD.

Applicants should have experience of working with individuals who have a learning disability and the appropriate skills, knowledge and experience in relation to autism, challenging behaviour and mental health within an integrated model of service delivery.
