Responsibilities
Conduct comprehensive assessments of clients' needs and circumstances
Develop and implement individualized care and support plans
Advocate for clients in accessing resources and services
Collaborate with healthcare providers, community organizations, and other stakeholders
Facilitate support groups and workshops to promote empowerment
Monitor clients' progress and adjust care plans as necessary
Maintain accurate and up-to-date documentation of client interactions and case notes
Requirements
Bachelor's degree in Social Work (BSW) or equivalent
Valid social work license or registration
Experience in adult social work or related field
Strong communication and interpersonal skills
Ability to empathize and build rapport with clients
Knowledge of relevant social services and community resources
Critical thinking and problem-solving abilities
Hours Per Week: 37.00
Start Time: 08:45
End Time: 17:15
Pay Per Hour: £28.03
