Responsibilities
Conduct comprehensive assessments of clients' needs and circumstances.
Develop and implement individualized care plans based on assessment outcomes.
Provide direct support and counseling to clients facing social and emotional challenges.
Advocate for clients' rights and access to necessary services and resources.
Collaborate with healthcare professionals, community organizations, and family members to ensure cohesive support.
Monitor clients' progress and make necessary adjustments to care plans as required.
Maintain accurate and confidential client records in compliance with legal and organizational standards.
Requirements
Bachelor's degree in Social Work (BSW) or related field; Master's degree preferred.
Valid social work license or eligibility for licensure.
Experience working with adults in social services or healthcare settings.
Strong interpersonal and communication skills for effective client interaction.
Ability to work independently as well as part of a multidisciplinary team.
Knowledge of community resources and social service systems.
Strong organizational skills and attention to detail.
Hours Per Week: 36.00
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £30.55
