Summary
Person requires support starting their day
Preferred days/times:
3.75 hours per week
Monday – Friday, 45 mins AM call five days a week
Tasks/duties:
Personal care (support to wash and dress), medication prompting, breakfast preparation
Client main diagnosis:
Fibromyalgia
Chronic fatigue Syndrome
Depression
Obstructive sleep apnoea
Diabetes
High blood pressure
Psoriasis
Degenerative hip/lower back
Fatty liver - NA
Macular disease
Adenomyosis
Chronic pain
Manual Handling
None required
Pets
The client has an assistance dog called Ralph
