Summary

We are looking for an Adult's Social Worker to join a Reablement Team.

This role requires a Social Work Qualification with a minimum of 2 years post qualified experience.

About the team

This is a fast paced team who work to optimise skills to support recovery from hospital. They ensure service users are able to live in their own homes if possible and in the least restrictive way. The team work alongside specialist care providers, focussing on multidisciplinary care with a strength based approach to each case to promote independence where appropriate.

About you

The successful candidate will be enthusiastic with strong communication skills and will have worked in a reablement or hospital based setting. A degree within Social Work (Degree/DipSW/CQSW) with a minimum of two years' experience working in an Adult's Social Work environment is essential. A valid UK driving licence and car is not essential but is preferred for this role.
