1 x part-time social care assessor (18.5 hours) to our Chester South and Neston Team.
We have exciting opportunities for those looking to join Adult Social Care as non-qualified social care assessors. You will have chance to work alongside Social Workers and Occupational Therapists, in line with the Care Act to provide high-quality, person-centred social care for adults, but for us it’s more than that; we support people to change and enhance their lives every day through the work undertaken by our talented and dedicated teams. This vacancy will be within the Ellesmere Port Community Team which is based at The Portal, Wellington Road, Ellesmere Port.

You will have:
NVQ Level 3 in Health and Social care or equivalent (Desirable).
Be required to undertake an enhanced DBS plus Adults Barred List (Essential).
Experience of working with vulnerable people and their carers.
Ability to communicate effectively with customers, carers and families.
Awareness of importance of multi-disciplinary working and an integrated approach.
Problem solver with good planning and organisational skills.
Ability to relate to other disciplines in a sensitive and professional manner.
Some knowledge of assessment and support planning within legislation.
Ability to cope positively with change and an ability to promote positive changes to make a difference.
You are confident, energetic and enthusiastic as well as sensitive and resilient.
Self-motivating individual with the passion to make a difference in adult social care.
