An exciting opportunity has arisen within SCFT/FOLS as a Senior Practitioner who will lead the multi-disciplinary Team, and promote recovery utilising trauma informed principles.

The candidate will provide expert advice, initiate and promote forensic mental health for service users who require robust risk assessment and management within the community.

You will be expected to safely manage a caseload and oversee the caseloads of other members of the team, for clients in inpatient settings and working towards discharge to the community across a specified geographical area, in accordance with Trust policies and procedures.

You will require excellent communication skills and work in collaboration with our colleagues in Humber, Navigo and North Lincolnshire Learning Disability Service.

You will require experience of working as a social supervisor for restricted patients, providing high-quality and timely reports to the Ministry of Justice as, knowledge of Safeguarding Children and Safeguarding Adults legislation.

You will need to demonstrate in-depth knowledge of the Mental Health Act 1983 (as amended by MHA 2007), particularly relating to care of patients in the community and have good report writing skills.

We will offer you clinical and line management supervision and promote career progression, continued professional development.

The responsibilities will include taking responsibility for individuals’ care and promoting recovery, including:
• Working closely and assertively with secure inpatient services to identify service users who are able to transition into the community, and co-produce plans to facilitate this
• Working collaboratively with service users in a recovery-focused, strengths-based way, identifying appropriate goals to promote their
• Ensure that the views and wishes of service users and carers are central to the care planning process and that their participation is
• Undertake care coordination duties in line with the Care Programme Approach to manage a caseload, working effectively with other members of the multi-disciplinary
• Ensure that specialist forensic risk assessments are undertaken collaboratively with service users and
• Provide effective, outcome- and evidence-based support and interventions to support recovery
