Summary


This is an exciting opportunity for an experienced clinician to join our Quality and Safety team and Pharmacy and Medicines team to support our outstanding organisation to respond to incidents, work to improve quality and safety across our health and social care services and support in delivering on medication safety.



This role presents a great opportunity for career development, enabling the post holder to gain real insights and experience of a corporate clinical role that can impact and improve on the quality of care provision in health and social care services.


To apply you must have a professional clinical degree level qualification and are currently on the NMC, GPhC, BASW, or HCPC register and have experience of undertaking investigations. thematic reviews and deep dive audits and have the skills to write comprehensive and accurate reports,








The post holder will have a key role in supporting the organisation to proactively work to reduce harms through quality improvement strategies and support the investigation and learning from incidents and other quality and safety concerns.

The post holder will utilise systems thinking, human factors understanding and just culture principles to address safety incidents to maximise the learning and supporting services to embed the identified safety improvements.

The quality and safety specialist will support the development of systems and processes to enable oversight of themes and tends from quality data (insight) and will work to equip service users, staff and partners with the skills and opportunities to improve service user safety (involvement) and designing and supporting programmes that deliver effective and sustainable change (improvement).

The post holder will support the review and development of policies and procedures to support safe and effective practice and will keep abreast of guidance and requirements nationally, regionally and locally to ensure best practice.

The post holder will be a lead investigator for safety incidents and other investigations where safety concerns have been identified. They will also support the clinical audit programme and quality assurance visits to gain assurance of quality and safety within the service in line with CQC and organisational standards.

The post holder will also collate assurance reports.

Management & leadership
• Deputise for the Quality & Compliance Manager as needed in their absence
• Support the Head of Quality & Safety and the Head of Pharmacy & Medicines in having oversight of all aspects of the Quality & Safety Agenda including providing expert clinical advice to clinical and non-clinical staff across the organisation
• Act as an expert Datix user able to create records, design and run reports and support and train others in these skills and competencies
• Work with the Head of Quality & Safety and the Head of Pharmacy & Medicines to undertake qualitative and quantitative analysis from governance activities, identify trends and highlight areas of good practice as well as concerns, working with colleagues to agree actions that aim to enhance standards and mitigate risk.
• Lead on all aspects of the review of incidents and harms
• Undertake investigations of incidents, complaints and concerns from themes and trends to identify service improvements using human factors and involvement to ensure co-production and effective outcomes.
• Write good quality objective reports that are factual and comprehensive, making recommendations for improvements.
• Undertake deep dive audits and clinical audits to review safety and support the annual clinical audit programme
• Plan and lead quality assurance visits /audits to review service quality and safety
• Maintain oversight of quality data flows and analyse themes and trends highlighting gaps and changes in trends to the relevant leads
• Provide support to QSLT in the production of High-quality reports and dashboards in line with the Division and organisational priorities
• Lead on the management of the Safety Alerts process to ensure effective review and distribution and responses. Leading task and finish groups to support organisational wide implementation of recommendations as needed
• Maintain oversight of the clinical and compliance audit programme to ensure it is on track, reporting on progress and outcomes.
• Oversee the completion of action plans arising from reviews, incidents, complaints and audits within the services providing support and assistance to help teams embed the learning
• Develop and review policies and procedures that reflect national and local legislation and guidance and support the Quality and safety team to maintain oversight of organisational compliance, supporting with redial actions where required
• Participate in all aspects of the quality and safety team activities to improve quality and safety across the organisation
• Provide training and support to teams including at induction on patient/service user safety and incident reporting and handling
• Provide and receive highly complex, sensitive and contentious information relating to incident and other investigations and present this information to teams and groups as appropriate.
• Keep up to date with developments both nationally and locally in relation to service user /patient safety
• Be aware of budget reporting processes and to support the QLT to validate invoices and undertake procurement and achieve savings in line with organisational requirements.
• Support the Head of Quality & Safety and the Head of Pharmacy & Medicines to respond to enquiries and request for information from the CQC and other parties external and internal to Provide
• Support the Head of Quality & Safety and the Head of Pharmacy & Medicines to collate and analyse complex data to support reporting of KPIs, achievement of compliance and assurance and completion of reports
• Work collaboratively and promote excellent working relationships with all colleagues across Provide and ensure that any grievances/disputes and complaints are resolved positively and speedily.
• Work closely with the designated quality and safety leads within integrated care system and partner organisations to strengthen the links and promote effective communication and partnership working


Clinical /Expert Skills
• The post holder will be required to complete and remain competent in patient safety specialist training as set out by NHS England along with any other national health or social care safety programmes relevant to their work and the needs of the organisation
• Be a Registered Health or Social Care Professional registered with the NMC, HCPC, GPhC or BASW
• Adhere to Provide policies and procedures and relevant national guidance
• Practice within the legal and ethical framework set out by professional codes of conduct and other relevant legislation such as the Care Quality Commission Standards and the Healthcare Act
• Ensure own continued professional development to maintain up to date and credible practice
• Provide expert advice / opinion and work positively to engage with people to deliver safe and effective outcomes
• Promote safety through empowering staff and working to identify and investigate incidents and complaints and to embed learning and improve practice and service delivery as a result
• Work proactively to prevent harms and safety incidents and near misses
• Support appropriate and accurate record keeping and promote good professional record keeping practice
• Safeguard, at all times, confidentiality of information relating to patients and staff.
• Be a good role model and raise the profile of quality and safety within Provide and keep abreast of national, and local good practice to improve service delivery and practice

Computer/Administration
• Be able to use Datix, e-mail, Internet, Word, PowerPoint, Excel, SystmOne/Access and Virtual conferencing systems confidently to facilitate good working practices

Communication
• Able to make operational judgements within complex and /or contentious situations or on receipt of facts, which require analysis and interpretation, to be communicated in a clear and concise manner, whilst managing professional differences of opinion
• Able to provide and receive highly complex, sensitive or contentious information, overcoming barriers to understanding when presenting information specialist information to patients, relatives and staff in a range of formats
• Able to effectively communicate with colleagues, peers, senior managers and clinical leads both internal and external to the organisation
• Take ownership for resolving conflict and engendering good and highly effective working relationships across the organisation, within your team and across the healthcare system
• Work to deadlines and demonstrate appropriate behaviour in stressful situations.
• Whilst managing time effectively display adaptability and flexibility
• Utilise influencing, negotiating and counselling skills as appropriate
