Crynodeb
Assist and perform routine tasks related to patient care as directed by the GP Partners To deliver Primarily Chronic Disease Management in Diabetes, Asthma, COPD including CHD, Hypertension, CKD, Rheumatology etc. Implement and evaluate individual treatment plans for patients with a long term condition. Routine immunisations/Childhood Immunisations: Understanding requirement for PGD and PSD. Travel immunisations Cytology/Swabs Wound care/minor injuries Urinalysis Venepuncture Ear Syringing Shared Care Prescribing Monitoring and Annual Review Primary Prevention Clinics Performance of ECG and Cardio-memo There will be a requirement to undertake home visits: Chronic Disease Management/Routine Immunisations/Venepuncture: The Practice Nurse will be expected to have a full driving licence and transport available.
