Summary


An exciting opportunity has arisen for an enthusiastic, dynamic, and motivated General Practitioner to join our Ageing & Complex Medicine Team working as an integral part of our multidisciplinary acute frailty team.

As the post holder, you will assist in the running of the Frailty SDEC and supporting your colleagues in managing the department.

We have a well-established reputation for postgraduate training and clinical research, as well as a strong dedication to providing high quality patient care. We are very keen to include GPwSI in our teams and improve the interfaces between primary and secondary care.

The ideal candidate will have knowledge of Care of the Elderly / Frailty, as well as an interest in general medicine or the motivation to learn and professionally develop while working in a supportive environment.



The role will have a specific remit for ensuring that frail older people in a ‘crisis situation’ receive expert, responsive, and safe care in a place closest to home, that best meets their needs.

Practice as an autonomous General Practitioner with special interest in Frailty, demonstrating advanced clinical competence and utilising highly specialised knowledge and skills to diagnose, plan, deliver and advise on programmes of care for patients.

To provide clinical leadership, diagnosis, management, and treatment for patients with specialist frailty and end of life care needs; act as an expert resource for medical, nursing, and other health care professionals across the Trust footprint; and be an active champion of high-quality frailty and older persons care both locally and across the region.

The post holder will:
• Be able to accurately assess a patient including history taking, examination, diagnosis, investigation, treatment, and where necessary referral onto multidisciplinary colleagues in a timely manner.
• Provide professional leadership, expert knowledge of frailty management, education, clinical advice, and experience to multidisciplinary team members as necessary.
• Maintain accurate clinical records record.
