Crynodeb
To lead and oversee the day-to-day management and co-ordination of the relevant sub-team within the Complex Cases Team and the process for undertaking NHS Continuing Healthcare Assessments, applying the Department of Healths National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care. To provide specialist advice and undertake clinical assessments as required with respect to the process for NHS CHC Assessment/Review for NHS Continuing Healthcare, to clients, health and social care professionals and other agencies, within the relevant and specific team, with the ability for cross cover across the directorate as and when required. This Job Description is generic and covers all Team roles and responsibilities.
