Due to the secondment of the post-holder, we are looking for an exceptional individual like you to join our Adult Social Care Hospital team who are making a daily difference to the lives of the people we serve.
This role is fixed term until 31st December 2025.
If you are enthusiastic, motivated and are up for a new challenge, then we have an exciting opportunity for you.
You will join an integrated multi-disciplinary team to promote independence and control to enable people to live the lives they choose to lead, whatever their age or stage of life.
You will assist in the management and development of an integrated multi-disciplinary team to provide high quality social work practice, to ensure staff understand and engage with adult social care values, vision and strategy. You will be working within the hospital and community to support people to maintain and regain independence following a hospital admission and be supporting with discharge planning and assessing for longer term support, considering alternative options to formal support.
You will have a professional social work qualification and be registered with Social Work England and provide practice and /or professional leadership, by supporting the development of research informed practice, delivering a performance and quality assurance function, supporting staff development and contributing to strategic development in the organisation.
You will champion and direct staff to engage and implement Adult Social Care values, vision and strategy to support people to have a life not a service.
