We are seeking a dedicated and professional Social Care Assessor to join an established Adult Services & Health team at Liverpool City Council. This is a critical role, supporting adults in need of social care. If you are passionate about making a difference and thrive in a fast-paced, collaborative environment, we want to hear from you.
- 35 hours per week (Monday to Friday, 9:00 AM - 5:30 PM)
- 1 month initial contract with possibility of extension after this
- £21.00 LTD per hour inclusive of holiday pay
Responsibilities:
- Assess the care and support needs of adults and their families, ensuring compliance with statutory frameworks.
- Work collaboratively with health professionals and other agencies to develop and implement care plans tailored to individual needs.
- Provide guidance to clients on accessing appropriate support and services.
- Maintain accurate and up-to-date records in line with council policies and procedures.
- Ensure all actions are in line with safeguarding principles and promote the independence and well-being of clients.
Requirements:
- Previous experience in a social care role, particularly within adult services.
- Strong understanding of relevant legislation, policies, and procedures in social care.
- Excellent communication and interpersonal skills to work effectively with clients, families, and multi-disciplinary teams.
- Ability to manage a diverse and demanding workload, prioritising tasks efficiently.
- A commitment to promoting equality and diversity in service delivery.
