We are looking for a motivated and experienced Band 4 Exercise Professional to join our small but dedicated outpatient team. In this role, you will be part of a multidisciplinary team and you will design and deliver exercise plans to improve the health and well-being of people with heart conditions. • Design and implement tailored exercise programmes
• Perform initial and follow-up physical assessments
• Plan and deliver group and one-to-one exercise sessions
• Deliver patient education on physical activity and exercising
• Monitor and assess patients' progress, and making adjustments if needed
• Liaise with physiotherapists, nurses, doctors and other healthcare professionals

Communication
• To effectively communicate complex information in a simple way to ensure patients and carers understand their condition and how to maximise their rehabilitation outcome. This requires using motivational techniques and positive, non-judgemental approach.
• To establish and maintain rapport with patients and carers.
• To overcome barriers and source solutions towards effective communication.
• To facilitate and be part of a well-coordinated multidisciplinary team for patient centred care.
• To maintain accurate, comprehensive, and up to date clinical documentation, in line with legal and service requirements.
• To communicate and/or escalate concerns to the appropriate clinician, including senior members of staff, consultants and GPs, via the most suitable means, email, telephone, letter.
• To establish and maintain healthy working relationships within but not limited to the team.
• To develop motivational strategies in order to support and inspire patients, carers and other staff members.
· To deliver presentations and in-service training and contribute to professional development forums.
· To promote the Cardiac Rehabilitation service and to ensure a consistent approach to patient care.
Analytical Tasks
• To undertake comprehensive assessment of patients with a range of clinical conditions, including those with a complex presentation, using analytical skills.
• To utilising a range of intervention skills and options to formulate a comprehensive, evidence-based individual management plan.
• To analyse and interpret clinical and social information in order to make appropriate recommendations for other professional intervention if required. To integrate this information into patient management plans.
• To evaluate the patients’ understanding in order to gain valid informed consent and have the ability to work within a legal framework. This includes patients who lack capacity to consent to treatment.
Planning and Organisational Skills
• To organise and manage a large caseload, with clinical priorities and delegate responsibilities to other staff as appropriate.
• To assist in the mentoring and training of support of staff and students.
• To co-ordinate and produce reports as requested for the area of work.
• To demonstrate a sound understanding of clinical governance and risk management and apply them at work.
• To deputise for other senior staff in their absence.
Physical Skills
• To lead a group or a one-to-one exercise session. This includes cardiovascular, strengthening, flexibility and balance exercising.
• To move and handle patients and pieces of equipment in a safe and effective manner.
• To maintain up to date training and knowledge of moving and handling, basic life support skills and immediate life support skills.
• To adhere to service policy including competence to use equipment.
Responsibility for Patient Care
· To be professional and legally responsible and accountable for all aspects of your own work, including the management of patients in your care.
· To ensure a high standard of clinical care and to assist staff to do similarly.
· To be responsible for identifying patient goals and development/monitoring of treatment plans.
· To work within professional and organisational guidelines.
· To strive to maintain standards of practice and undertake regular training as required.
· To be responsible for the safe use of equipment by patients under your care.
· To provide relevant advice, to other members regarding the management of patients.
· To participate in referral, assessment, intervention, and evaluation in order to meet patients’ needs.
Policy and Service Development Implementation
• To assist in identifying, maintaining and monitoring quality standards for clinical practice within the service.
• To use appropriate networks to share information, research and good practice examples.
· To demonstrate clear understanding of the role of national agencies and legislation and systems for improving the quality of care and work within these frameworks to deliver a high quality physiotherapy service.
· To ensure that work is conducted in accordance with organisational / service policies and guidelines and within Health and Safety Regulations.
• To contribute to training and development programmes in the current service area
Responsibilities for Financial and Physical Resources
• To be aware of budgetary constraints.
• To assist the line manager in the delivery of a cost-effective service and be aware of performance measurement criteria.
• To be involved in the collection of appropriate data and statistics for departmental audit purposes.
Continued Professional Development Responsibilities
• To participate in the supervision/mentoring of other members of staff as well as students.
• To contribute to the training and development of others as and when required to improve service provision.
• To take responsibility for own development and actively participate in supervision.
• To maintain own continuous professional development (CPD).
• To be an active team member and participate in in-service training programme by attendance and involvement at staff meetings, training sessions, external courses and reflective practice.
Responsibilities for Information Resources
· To be responsible for collating data as per clinical area.
· To be responsible for the recording of clinical information in relation to patients and clients and their treatment programmes ensuring confidentiality is maintained at all times.
Responsibilities for Research and Development
• To actively search out and incorporate evidence into practice.
• To contribute to the measurement and evaluation of the service. This may involve the use of audit and outcome measures, in association with other services.
