Summary

We are currently looking to appoint a Support Planner, which will be working under the general direction and supervision of the Business Support Planner Lead and to work as a member of the re enablement and peripatetic service to ensure the efficient and appropriate day-to-day delivery of service inclusive of reablement, long term care, extra care, night care and day services



You will:

Effectively deal with queries and complaints
Able to communicate effectively both orally and in writing
Ability to keep accurate records and provide management information as appropriate.
Ability to use and maintain information technology systems. Individual time management and prioritising essential workloads
Organisational and team working skills.


Knowledge

Of issues relating to older people and physical disabilities.
Of health and safety issues in this area.
Understanding the need for confidentiality


Daily tasks/duties and responsibilities

To allocate work, through use of the re enablement IT Management Systems, to Home Care Staff to maximise effectiveness and efficiency within the service, including changes in service delivery plans, any absences of service users and of staff.
Advanced planning to promote and ensure continuity of re enablement to service users and coordinating staff rotas and work schedules on a daily basis, to cover annual leave, sickness and training.
In liaison with reablement outreach practitioners and managers make decisions regarding changes to service delivery and communicating effectively with service users, staff and managers regarding these changes to ensure calls are not missed.
To ensure safe working practices by compliance with health and safety legislation and guidance.
To monitor and inform the reablement coordinator, through use of the electronic management systems, of changes to actual service delivered against the service user's individual service delivery plan.
Input changes to service delivery plans.


To action care plan changes also and to inform or discuss with all affected parties. I.e. service users and support staff, and follow up with confirmation letter and document all discussions


Essential requirements:

English and maths equivalent to GCSE pass level. CLAIT, IBT Level II or equivalent experience.


Desirable but not essential:

NVQ 3 in customer care
