Summary
The implementation of Health Promotion and disease management strategies (in line with the National Service Frameworks) and provision of a full range of nursing services to the patients of the practice .Chronic Disease Management as required by the practice.Accountable to Clinical Partners , Managerial to Practice Manager . The completion of health checks, health screening, immunisation and vaccination programmes.To provide a service to patients with injuries or conditions requiring a dressing.
