Summary


Ward 4 is a 30 bedded unit, offering stroke and elderly rehabilitation.

Many of our patients have complex needs.

You will be required to work under direct supervision of the qualified nurse to provide high quality, safe patient care and treatment, and to work with the Multi-disciplinary team to promote patient rehabilitation.

Assist patients with all activities of daily living, including washing, dressing, bathing, mobility, continence, hydration and nutrition.

Undertake clinical observations

Blood glucose monitoring

Mouthcare assessment and intervention

Nutritional assessment, ensuring high quality nutritional support, fluid balance.

Moving and handling

Maintain privacy, dignity and confidentiality.

Communication with patients, their family and carers

We manage three major locality hospitals at North Tyneside, Wansbeck and Hexham, plus a number of smaller community hospitals and clinics from Tynemouth to Berwick on Tweed, in addition to our state-of-the-art Northumbria Specialist Emergency Care Hospital, the first of its kind in England. We also care for people in their homes and provide services from facilities in local communities such as health centres. We give people greater choice and control over their care to help them to live independently at home and avoid hospital admission where appropriate. Undertake a range of delegated clinical care duties in the ward environment under the direct supervision of a Qualified nurse. Records patient observations and changes to patient clinical conditions.

To work under the supervision of a qualified nurse and assist in the implementation of both direct and non-direct patient care.

To provide and promote high quality care under the guidance of a qualified healthcare professional.

Work with the MDT to promote rehabilitation and monitor patient goals.
