Main Duties and responsibilities: Programme Delivery and Management: Coordinate and oversee the delivery of the FaME programme in Eastwood (24-week programme). Support instructors in setting up and delivering the FaME programme. Delivering the existing OTAGO class in Hucknall. Set up and deliver new OTAGO classes in Arnold and Cotgrave, ensuring each runs effectively for 12 months. Engage with community groups to ensure effective promotion/signposting/referrals to and from offers they already provide e.g. seated exercise; Tai-Chi; walking groups, etc. Data Management and Reporting: Capture accurate data from outcome measures and goal-setting targets in line with the commissioners specifications. Monitor participant progress and provide ongoing support throughout the programme. Prepare detailed reports and present data during performance review boards and contract reviews. Support the ongoing evaluation of this initiative as required. Client Support: Provide one-on-one and group support to programme participants. Utilise motivational interviewing and positive psychology techniques to encourage sustainable behaviour change. Monitor and track client progress, offering personalised advice and adjustments to their wellness plans. Collaboration and Networking: Establish and maintain strong relationships with community stakeholders. Work collaboratively with the multi-disciplinary team to enhance service delivery and client outcomes. Liaise with GPs and other relevant healthcare professionals to ensure comprehensive client care. Establish strong relationships with relevant local authority colleagues i.e Health and Wellbeing officers, Local Leisure Centre providers, and Adult Social Care teams. Administrative Duties: Maintain accurate records and data in accordance with organisational policies and procedures. Prepare and contribute to reports on programme outcomes and impact. Ensure all activities and interventions comply with relevant guidelines and standards. Continuous Improvement: Stay up to date with the latest research and best practices in health promotion and community wellness, specifically falls prevention. Contribute to the development of new resources and materials to support programme delivery. Participate in training and professional development opportunities to enhance skills and knowledge. Skills & Competencies required: Experience in implementing and delivering community health programmes. Strong organisational and coordination skills with attention to detail. Excellent communication and interpersonal skills. Ability to engage and motivate individuals and groups. A proactive approach to problem-solving and programme management. Competence in using digital tools and platforms for virtual delivery and client engagement
