Summary
We particularly welcome applications from Black, Asian and minority ethnic candidates and those with lived experience, as they are under-represented within the Trust.
NBT and UHBW were awarded a place on the NHSE Cancer Improvement Collaborative (CIC) programme. The local project team is comprised of committed people from both trusts and has support from colleagues within the wider ICS. The project focuses on improving the quality of reasonable adjustment provision for people on cancer pathways within the BNSSG area. Many people with cancer are treated by both trusts which adds a level of complexity for people requiring reasonable adjustments. This may include, but is not limited to, people with a learning disability, autism, dementia, severe mental health condition, hearing impairment, physical disability, and includes individuals with experience at the intersection of multiple conditions.
We need a passionate & highly organised individual to join the team and drive forward this important work within UHBW, scoping the current practices around reasonable adjustments. The role is a great opportunity for someone who wishes to be involved in an exciting project which will help reduce health inequalities and enhance the experience of people with cancer. This provides a fantastic opportunity for you to contribute to improving the overall experience of cancer care at UHBW and work within a friendly & supportive team with an interdisciplinary approach to delivering personalised care
• Work closely with the project team and other stakeholders to scope current practices around reasonable adjustments, which will feed into the wider BNSSG Cancer Improved Collaborative projects.
• Hold responsibility for the creation and management of a resource database, relating to supportive organisations. Guiding people through the use of self-management resources
• Utilise outstanding communication skills and initiative to work across all elements of the cancer pathway
• Identify & escalate associated challenges to the project team
• Create and manage a patient tracker, identifying those people affected by cancer, that require reasonable adjustments.
• Support the promotion of the accessible information standard (AIS, 2016) within UHBW, including the new learning module
• Encourage the operationalisation of the AIS into cancer services, with focus on the education of those involved within the oncology pathway.
